Position Details:
Clinic Nurse, Western Community Health Resources
Part-time 20-29 hours a week, Permanent, Hourly, Flexible schedule, Benefited position

Must be willing to travel to Alliance, Gordon, and Crawford.
Position Summary:
Assisting family reproductive health, Immunization, and WIC patients. Case management, service coordination, outreach and health education and follow up for health projects.
Primary Job Duties:
- Obtains and reviews health histories, performs screening and assessment in home, office, school and other environments as necessary and develops service plan for provision of medical services to clients.
- Completes client forms and maintains clinical records.
- Explains projects and services to eligible clients.
- Collects client fees and is responsible for reconciling income daily, in conjunction with Program Assistant/Office Manager.
- Issues vouchers and/or specified food packages for services.
- Issues oral contraceptive methods to clients.
- Provides agency defined case management services for clients; monitors needs, services provided to meet those needs and client compliance. Ensures that patient follow-up is performed per phone, letter, or telehealth as appropriate.
- Provides intensive and extensive education on nutrition, pre-natal care, reproductive health, immunization, and other health care needs associated with WCHR programs.
- Develops relationships, coordinates and collaborates with agencies within the region that will benefit the organization and people served. Maintains personal contacts within those organization and agencies.. Makes referrals to other health and human service organizations.
- Attends meetings and conferences as approved by supervisor.
- Provides basic lab screening to include UA's, HGB, B/P, Ht/Wt., pregnancy tests, diabetic and cholesterol blood tests, HIV testing and immunizations and STD screenings.
- Collect specimens requested by provider, correctly label and record in appropriate computer programs. (Ex: FPAR, NextGEN, Journey, Elirt and NeSIIS)
- Assist answering phones and making appointments as needed.
- Be familiar with Immunization Programs and be able to look up patient record and make appointments as needed.
- Promotes WCHR programs through consistent community activities and outreach.
- Is familiar with WCHR programs in order to refer individuals.
- Attends meetings and trainings to update knowledge of materials and information related to client education in all program areas.
- Ability to independently carry out decision making functions consistent with program guidelines and policies.
- Ability to prepare, develop and present educational presentations and/or classes to the public.
- Ability to administer injections to infant, children and adults; operate hemocue machine, scales, sphygmomanometer, and cholesterol/diabetes screening equipment.
- Ability to collect and maintain data gathered from outreach, meetings, presentations and client contacts to complete regular reporting and grant requirements.
- Participates in program planning activities.
- Special duties/projects as assigned for program development relating to overall purpose of project and organization and maintains confidentiality as described by professional practice.
- Occasional weekend or evening community projects.

Education:
- Ability to practice in Nebraska as a licensed RN or LPN.
- Case management and health education skills.
- One to three year or experience in nursing.
Experience:
- Experience in community based services or working with low-income people is helpful.
- Diverse clinical experience is helpful.
- Knowledge of agency mission and individual program objectives.
- Working knowledge of basic office methods and procedures.
Requirements:
- Access to reliable transportation, valid Nebraska driver’s license and at least the legal minimum car insurance.
- Available to travel and work flexible schedule.
- Moderate level of computer skills.
