Full-time or part-time position, Monday-Friday, hourly.
On-call five days a month (subject to change). Benefited position after 60-day waiting period.
Position Summary:
Works under the direction of the Home Health Clinical Manager and the Hospice Director with supervision from the Registered Nurses. The LPN provides direct nursing care, personal care and educating family/caretaker to assume care of patient. Coordinates with professional Home Health & Hospice staff and works under the general direction of a registered nurse. Utilizes the Electronic Medical Records.
- Follows the Plan of Care providing Skilled Nursing ordered services as a person knowledgeable in scientific and technical principles, and drugs and therapeutics.
- Provides services that are ordered by the physician, as indicated in the plan of care, as a person knowledgeable in scientific and technical principles, and drugs and therapeutics.
- Evaluates patient / caregiver / family response to plan of care and documents in the patient’s medical record. Notifies RN Case Manager, physician(s), family, caregiver(s) and appropriate team members of unfavorable responses per agency policy.
- Evaluates patient / family response to plan of care, living conditions in regards to safety of caring for patient in the home.
- Functions and participates as a team member demonstrating coordination and collaboration with, and responsibility to RN Case Manager, patient/family where applicable, caregiver, physician and interdisciplinary team.
- Collaborates with RN Case Manager and the interdisciplinary team. Documents this through the Electronic Medical Record ‘notes’, documented phone contacts and attendance at case conferences (or pre-conference).
- Communicates with the physician who is responsible for the plan of care and other health practitioners (as appropriate) related to the current plan of care.
- Responsible for patient and caregiver education regarding disease process, pain & symptom control, nursing and related needs to ensure a timely discharge.
- Assist RN Case Manager to coordinate and monitor care for continuity of services between home care and inpatient settings. Initiates needed referrals to other disciplines and/or community resources.
- Assumes responsibility for economic use of equipment and supplies and assures proper charges are made to patients. Obtains needed supplies and equipment to provide care.
- Observes and documents progress, symptoms, and reactions with accuracy and thoroughness in the patient in the patient’s record per agency policy.
- Recognizes emergencies and institutes emergency procedures.
- Has a thorough understanding of the regulations concerning patient care and the philosophy and goals of the Home Health & Hospice and is able to interpret these to the public.
- Adheres to on-call policy and responds appropriately to request for care by patients while on call.
- Treats patients and their families with respect and dignity. Identifies and address psychosocial needs of patients and families.
- Honors patient rights and will immediately report any evidence of abuse, neglect or exploitation of any patient per agency policy.
- Participates in the Agency Emergency Preparedness Plan.
- Follows infection control and prevention procedures.
- Participate in the Total Quality Performance Improvement program in the Home Health & Hospice Program and assist with chart audits as directed by the Director of Home Health & Hospice.
- Participates in continuing education, assist in staff in-services and maintains competence in performing nursing skills and procedures by attending in in-services, committees, etc. and shares knowledge with others.
- Participate in hospice Volunteer orientation as directed by the Director of Hospice.
- Provide Bereavement Services as directed by the Director of Hospice.
- Maintains a harmonious relationship with staff members and strives for good communication at all times.
- Will have an insured automobile for work and a valid Nebraska driver’s license

Educational Requirements & Experience:
- Graduate of an accredited school of nursing.
- One year of Med-Surg, Rural Health, Home Health, Hospice or Nursing Home care, preferred.
