The LPN contributes to the ongoing resident assessment and plan of care for the resident by observation and data collection.  The LPN implements, evaluates and documents resident care as directed by the RN.  

30-35 hrs a week 2:30-9:30pm

  • Responsible for the day to day management of assisted living but not limited to:  continuity of care and safety of residents, monitoring resident condition and evaluating needs, staffing and scheduling medication and bath aides, providing in-service opportunities for medication and bath aides, maintaining equipment and evaluating environment for safety, and supervision of medication and bath aides, as well as Job Corps students.

  • Demonstrates clinical skills in providing resident care through collection of subjective and objective information about resident: prioritizes delegated cares for residents, evaluates nursing needs and assesses safety, emotional, and social needs of resident, implements interventions when needed for resident safety, evaluates resident responses to medical treatment and need for additional care, and observes, reports and documents change in condition of resident.

  • Utilizes communication skills effectively when interacting with residents, families, staff, and providers: takes physician orders correctly and orders medication promptly, acquires and disseminates pertinent information, interacts professionally as a liaison between resident, family and providers, communicates with family and POA as needed for resident needs, communicates with medical providers and pharmacists verbally and in writing, communicates with a variety of visitors, families, and friends of residents, and is a positive representation of nursing and PPL/CCH in hospital, AL and IL, and in the community.

  •  Practices are based on PPL Nursing and Assisted Living policies and procedures: updates and maintains compliance with state regulations and changes, keeps updated on all regulations pertaining to nursing/medication aide/bath aide, maintains current license without restrictions, and maintains current knowledge through conferences, workshops and seminars and/or in-services.

  •  Assists with the orientation and education of staff and students: participates in orientation of new staff, shares knowledge with other staff and students as appropriate, supports an environment conductive to professional practice by functioning as a role model and resource person, performs yearly competency testing of new & existing staff, and perform annual staff reviews/evaluations.

  • Participates in TQI studies by contributing to quality of resident care through TQI studies, chart audits, and cost/benefit analysis.

  • Attends and participates in approved/requested committees and meetings, including self-scheduling committee and monthly nurses meeting, and demonstrates flexibility in adjusting to variable work load and job duties.
