Position located at Western Community Health Resources (WCHR)

Full-time 30-40 hours/week, Days, Benefits eligible

Position is eligible for work force stabilization plan bonuses that are received quarterly. 

This position would cover the counties of Dawes, Sheridan, Northern Sioux, and Box Butte.

Position Summary: 

Identify, triage, and manage patients with medical and behavioral health problems within the primary care setting; provide skills training through psychoeducation and patient education strategies. Is responsible for providing comprehensive, integrated mental health care to patients in the community. 

Duties to include but not limited to:

  • Provide counseling/assessments with diagnosis (both in person and via telehealth) for WCHR patients referred by providers and WCHR programs. 
  • Participate in Clinical Behavioral on-call coverage schedule. 
  • Conduct psychosocial evaluations and provide therapeutic interventions including crisis counseling to individuals, groups, and families as needed. 
  • Clinical assessments, treatment, case management, consultation, referral, and education according to the individual's professional discipline. 
  • Evaluate individuals and family systems. 
  • Work with primary care teams. 
  • Develop, implement, and maintain partnerships with other community providers.

  • Licensed Independent Mental Health Practitioner (LIMHP), Advance Practice Registered Nurse Practitioner (APRN), Provisional Licensed Mental Health Practitioner (PLMHP)
  • Current Nebraska license required. Will consider provisional license.
