Position Details:
Service Coordinator - WCHR
This position would cover the counties of Dawes, Sheridan, Northern Sioux, and Box Butte.
Full-time, Days, Travel

Position Summary:
Responsible for identification, referral, and coordination of services for adults, children, and/or families. The Service Coordinator also serves as the single point of contact in helping clients gain needed services and assistance. Crisis Response Triage assessment skills.
Duties to include but not limited to:
- Secures information releases to facilitate sharing of information and notifies referral source that contact has been made with the individual or family.
- Maintains ongoing communication with referral source and other contacts as requested by the individual or family.
- Identifies individual or family needs, strengths, and priorities on a continuing basis.
- Advocates for the individual or family to ensure priorities remain the driving force behind the case plan.
- Assis the individual or family in forming the support team and gaining access to services.
- Assists the individual or family in identifying gaps in services and relaying that information to the case plan and other agencies.
- Completes, updates, and disseminates case plan documents, as appropriate following confidentiality and time frame guidelines.
- Serves as the mediator for addressing and managing conflicts among individuals and/or families, agencies, and service providers.
- Maintains records of required data and confidential records.

Qualifications:
- Any combination of training and/or experience that will enable the incumbent to possess the required knowledge, abilities, and skills.
- A general qualification guideline for positions in this class is post high school course work/training in social/behavioral sciences, education, or counseling.
- Recommended to have experience working with families with special needs, young children, infants and toddlers, youth and adults.
Experience:
- Two years experience in a Health and/or Human Services field, or 24 semester hours of post high school education in Behavioral Science, Social Science, or closely related field.
- Or any combination of education and experience that equates to two years in Human Services, Behavioral Sciences, Social Sciences, or closely related field.
- Must have experience working with families of children with special needs, young children, infants and toddlers.
- Experience with a broad variety of community agencies that provide services to adults, young adults, families, or young children with disabilities.
- Experience working with treatment of individuals with a mental health diagnosis.
- Experience working with culturally diverse individuals and/or families.
Education:
- Preferred, Bachelor's Degree or higher in Psychology, Sociology, Social Work, or related Human Service field.
