- Student_Volunteer Packet (PDF below).
- Immunization Records
- Parental consent - if under the age of 19, parental/legal guardian consent is required.
- Background (if this is for a internship or clinical rotations).
- Once a completed packet is received, you will be contacted regarding scheduling, availability, and details.
