Student Job Shadow & Intern Application
The following requirements must be completed before a student is approved to participate in a job shadow or internship experience:
Please complete the Student Application and submit it to Human Resources or Clinical Education via email at:
lacy.cleveland@chadronhospital.com or heidi.wess@chadronmd.com
Prior to shadowing, you must provide and/or complete the following:
- Student/Volunteer Packet (PDF available below)
- Immunization Records
- Parental/Legal Guardian Consent – required for students under the age of 19
- Background Check – required for internships or clinical rotations
Once all documents are received and your packet is complete, we will contact you to discuss scheduling, availability, and next steps.
