Health Insurance - Regional Care Inc. (RCI)
This is a self-funded benefit plan coming within the purview of the Employee Retirement Security Act of 1974 ("ERISA"). The Plan is funded with employee and/or employer contributions. As such, when applicable, Federal law and jurisdiction preempt State law and jurisdiction.
Eligibility:
Eligibility Requirements for Employee Coverage. A person is eligible for Employee coverage from the first day the employee:

is a Full-Time, Active Employee of the Employer. An Employee is considered to be Full-Time if he or she works at least 30 hours per week and is on the regular payroll of the Employer for that work.

is a Part-Time, Active Employee of the Employer. An Employee is considered to be Part-Time if he or she works at least 20 hours per week and is on the regular payroll of the Employer for that work.

completes the employment Waiting Period of 60 consecutive days as an Active Employee. A "Waiting Period" begins the first day of employment as an eligible Employee
Dental Vision Insurance - Ameritas
Dental

Type I benefits start at 70% and increase 10% at the beginning of each calendar year until 100% is reached. You must have at least one dental procedure performed each calendar year while insured by this plan to qualify for the increased benefits.
If you fail to have a dental procedure performed in any calendar year, your benefits will revert to 70% and you will be eligible for 10% annual increases from that point.

Vision
FOCUS is a vision product offered by Ameritas throughVSP (Vision Service Plan).

Retirement Program - One America - AUL
Fully administered retirement program established August 1, 1974, which falls under Sections 401(a) and 403(b) of the Internal Revenue Code.

Employer Contributions - Attainment of age 21, completion of 1 year of service and 1,000 hours of service per year.
Base Term Life, LTD and AD&D - UNUM
CCH&HS is pleased to pay 100% of coverage for:
Employee: 1x basic annual earnings to a maximum of $255,000
Eligibility: All active employees working a minimum of 30 hours per week.
Waiting Period: 1st of the month following 90 days of continuous active Full Time Employment.
Voluntary Term Life and AD&D - UNUM
Eligibility: All active employees working at least 30 hours per week and their eligible spouses and children (up to age 19, or to age 26 if they are full-time students.)
Voluntary Benefits - METLIFE
Accident - For any accident on or off the job anytime you seek medical treatment, large or small
Cancer - Pays you tax free cash in the event of cancer or other specified diseases. Wellness benefit paid each year for screening tests like pap, mammogram, and PSA.
Critical Care and Recovery - Helps pay expenses related to heart attack, stroke, coma, paralysis, third degree burns and more. Provides a direct, immediate tax free cash benefit upon diagnosis
Hospital Advantage - Most plans don't pay all the costs of your treatment. Supplemental health insurance can help protect your hard-earned savings, and it can help you cover some of the costs associated with hospital care.
Eligibility:
All active employees working at least 30 hours per week for Full Time. Must sign up with the representative during the plan year visits around March.
Flexible Benefits Program
Unreimbursed Medical Account
You can use pre-tax dollars to pay for your family's expenses that are not reimbursed by your health insurance plan. IRS code section 213(d) defines eligible expenses and there are examples included in this packet. You can elect to contribute up to the maximum set by your employer. However, the IRS does not allow you to change your elected amount during the plan year unless you have a qualified event—marriage, divorce, birth, adoption, death of dependent, or change in employment status. If there is money left in your account at the end of the plan year or if you terminate employment, please refer to your SPD (Summary Plan Description) to determine your claim filing deadlines.
Dependent Care Account
Individuals can set aside pre-tax dollars for day care expenses for dependents. You (and your spouse, if married) must be working, looking for work (must have income during the year), or attending school full-time. The household limit for a family is $5,000 (this includes child care subsidy amounts) and if you are married filing your taxes separately the maximum is $2,500. If there is money left in your account at the end of the plan year or if you terminate employment, please refer to your SPD (Summary Plan Description) to determine your claim filing deadlines. You may want to contact a tax advisor to determine whether the dependent care account or the federal child care tax credit program is better for you.

All active employees working a minimum of 30 hours per week. Coverage is every calendar year and sign ups are due in mid December for the upcoming year of coverage.
